Home Care (Personal Care & Support Services)

  • Personal Care Assistance
    Helps with activities of daily living—bathing, grooming, dressing, toileting, meal prep, laundry, and household tasks—based on an approved care plan
  • Skilled Nursing & Advanced Supportive Care
    Enables licensed nurses (RNs or LPNs) to perform medical tasks—notably assessments, monitoring, medication prep, wound care, and oversight of aides—on an intermittent basis to help participants remain safely at home
  • Respite Care
    Offers short-term caregiver relief either in-home (2–7 hours) or in facility-based/residential settings, included when needed to prevent institutionalization

Case Management Services

Our certified case managers serve as the guiding hand for each client’s care journey. Their role includes:

  • Comprehensive in-home assessments
  • Development of a person-centered care plan
  • Coordination of all approved waiver services, including but not limited to:
    • In-home care
    • Adult Day Health
    • Home-delivered meals
    • Therapy services
    • Assisted Living placement and coordination
  • Ongoing monitoring and support
  • Advocacy for client needs and service quality
  • Annual reassessments and plan adjustments
  • Support with transitions between care settings (hospital, rehab, or long-term care)

Consumer-Directed Option (CD‑PASS)
For clients who qualify and prefer a community-based environment, our case managers may help coordinate CD-PASS option on the service plan. Participants can self-direct personal care—

Additional In‑Home Support that may be added to your plan:
Includes home-delivered meals, prescription coordination, home modifications (ramps, grab bars), specialized medical equipment/supplies, personal emergency response systems (PERS), adult day health, remote supports, and even hospice care
